If you choose to use consent as the basis for collecting and processing survey response data, Snap allows you to provide potential respondents with the relevant information that they need to give informed consent, and to configure your survey to obtain the respondent\u2019s consent to proceed with the survey.<\/p>\n<p>Respondents choose whether to give consent by answering a specific question at the beginning of the survey.&nbsp; If they choose not to provide consent they are prevented from completing the survey.<\/p>\n<h4>This worksheet shows you how to:<\/h4>\n<p><a href=\"#o33211\" target=\"_self\" rel=\"noopener\">1: Provide potential respondents with the relevant information<\/a><br \/>\n<a href=\"#o33212\" target=\"_self\" rel=\"noopener\">2: Create a Multi Choice style consent question<\/a><br \/>\n<a href=\"#o33213\" target=\"_self\" rel=\"noopener\">3: Set the question as a mandatory response<\/a><br \/>\n<a href=\"#o33214\" target=\"_self\" rel=\"noopener\">4: Preview the survey to test it<\/a><br \/>\n<a href=\"#o33215\" target=\"_self\" rel=\"noopener\">5: Keep a record of consent<\/a><\/p>\n<h4><a name=\"o33211\"><\/a>1: Provide potential respondents with the relevant information<\/h4>\n<p>You need to ensure that the consent that you obtain from respondents is \u2018informed\u2019, meaning that respondents have been given \u2018fair processing information\u2019 and have been informed of all aspects of the survey project that are relevant to their decision to participate. This is commonly done by providing potential respondents with a privacy or information notice. You can present this information by including it in the instruction question type field at the start of your survey. You have the option of providing this information in a layered format by providing a summary of the essential information in the instruction question type field, followed by a link to your full privacy policy or notice for more detailed information. You are also able to include a link to the Snap Surveys\u2019 Privacy Policy, available at <a href=\"https:\/\/www.snapsurveys.com\/survey-software\/privacy-policy-us\/\" target=\"_blank\" rel=\"noopener\">https:\/\/www.snapsurveys.com\/survey-software\/privacy-policy-us\/<\/a>, for information as to how we act as your data processor.<\/p>\n<p>For instructions on how to insert a link, see our <a href=\"https:\/\/www.snapsurveys.com\/help\/#18132\" target=\"_blank\" rel=\"noopener\">Help page<\/a> on \u2018Inserting HTML links and codes\u2019.<\/p>\n<h4><a name=\"o33212\"><\/a>2: Create a Multi Choice style consent question<\/h4>\n<ol>\n<li>Open your survey in the Questionnaire Window and Design Mode&nbsp;<img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-26695\" src=\"https:\/\/www.snapsurveys.com\/support-snap11\/wp-content\/uploads\/2018\/03\/design.png\" alt=\"\" width=\"32\" height=\"31\"> (Default).<\/li>\n<li>Click into the first title or question field at the top of your questionnaire.<\/li>\n<li>Click&nbsp;<img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-full wp-image-22699\" src=\"https:\/\/www.snapsurveys.com\/support-snap11\/wp-content\/uploads\/2016\/07\/Plus.png\" alt=\"plus | add\" width=\"24\" height=\"22\">&nbsp;to create a new question.<\/li>\n<li>Select the dropdown list on the toolbar where it says <strong>Single Choice<\/strong> and change it to <strong>Multi Choice<\/strong>. The shape of the response boxes changes from a radio button to a square.<\/li>\n<li>Click on the field below and Press [Ctrl] + [Shift] + [B] to insert a page break above your new question (alternatively click the right\u2013hand button on the mouse and select Break and Page Break.<\/li>\n<li>In the area marked <strong>Click here for text<\/strong>. You can set the question and the explanation to include whatever information you as data controller consider appropriate for your survey, the type of data that you propose to collect, and the uses that you propose to make of the survey response data.<\/p>\n<p>For example, where special categories of personal data will be collected (e.g. gender and ethnicity data) and explicit consent is required, you may wish to include specific wording to make it clear that the respondent is willing to provide such information. The Preview will open a new window and display your survey as it would appear in a web browser immediately after it has been published.
